The purchaser will need to complete and fax or mail in the "Authorization Form"
and/or Agreement for the optimization service to start. The purchaser and/or

15. SERVICE REFUSAL - We reserve the right to refuse service.
16. PAYMENT METHOD - We accept Visa, MasterCard, American Express, Discover, and
any debit card with a Visa or MasterCard logo on it. Due to the automated nature
of our service we do not accept checks, money orders, cashier's checks, cash, or
any other form of payment.
17. MISCELLANEOUS - This Agreement shall all be governed and construed in
